Do you know that doulas support cesarean birth? As Ottawa Doulas, our goal is to support families in all types of births. Sometimes, due to circumstances beyond a family's control, or by choice, this includes cesarean birth! Whether it's an unplanned or planned cesarean, doulas can help make cesarean birth as positive an experience as possible.
Prenatal Conversations
Thinking about, and talking through what to expect during a cesarean, is an important part of prenatal care. Whether it's to learn about why unplanned cesareans happen and how often they happen, or to talk about what to expect for a planned cesarean, opening up conversation during prenatal visits allows for families to prepare for all possible birth pathways. Also, talking about cesarean early helps families be informed an empowered about their choices when it comes to cesarean birth. While it may feel uncomfortable talking about an unplanned cesarean, it's worth it for the sake of helping families feel as supported as possible for every birth potential.
Planning Ahead
Whether a cesarean is planned or unplanned, creating a cesarean birth preference list is always helpful! Knowing what choices are available can help each family plan a cesarean birth that feels individualized and family centred. Nurtured Birth Ottawa doulas share a list of cesarean birth preference options with each family to help guide them to understand what their options are.
Gentle Cesarean Birth
The term "Gentle Cesarean" is becoming more popular as families understand their choices when it comes to surgical birth. The aim of a gentle C-section is to create as nurturing a birth experience as possible by bringing comfort elements into the operating room.
Supportive elements to be considered for a gentle cesarean include but are not limited to:
Playing music during the birth
Asking the surgical team to avoid small talk during the surgery
Asking for one arm free to hold your spouses hand
Requesting the baby be delivered slowly and respectfully
Photos being taken so that the family can preserve their birth memories
Delayed cord clamping
Skin to skin with mother or spouse as soon as possible
Gentle cesarean improves the overall experience of surgical birth and reduces birth trauma. We encourage each family to have a discussion about Gentle Cesarean birth with their doula.
Doula Support in the OR
Ottawa cesarean birth policies are different depending on which hospital you visit. Unfortunately, across the board, most families will experience pushback from their medical team with regards to having a doula provide support in the operating room. Having these conversations with your provider early, and strongly advocating for doula support at the time of the cesarean can help. Ottawa Doulas are working hard to shift the policies around doula support in the OR, but the biggest way to create change is for families to keep asking and keep strongly advocating for it to happen!
When doulas are not invited into the operating room, we show up to support you before the surgery. We help the family get settled in, feel ready, remember their birth preferences, answer any questions they have, and stay calm and centred.
If we are invited into the operating room, our doulas provide emotional support, information, physical support (there is nothing like a reassuring hand on your forehead, and a reminder to breathe through the more challenging parts of surgery), take photos of the family with their new baby, and more!
Following the cesarean, the doula will meet the family in recovery (when permitted) to support early recovery, skin to skin, bonding, and baby's first latch. We also helps get the family settled into the postpartum unit, ensure they hydrate and have a few small bites of food, and make sure any personal belongings they have are close by and easily accessible.
Postpartum Care
Our Ottawa Doulas are also able to offer guidance around recovery and healing from cesarean. The fourth trimester can look very different following a surgical birth. Doulas are available for questions and in person support, including helping you set up your living space to ensure you are getting the rest, nutrition, and physical interaction you need without overdoing it!
Many Ottawa Doulas also offer in person postpartum support as either an add on service or part of their birth package. Following a cesarean birth, a postpartum doula may help in any of the following ways:
Physical Recovery Support
Encouraging rest and protecting the healing window
Assisting with safe movement (getting in/out of bed, standing, sitting)
Gentle reminders around pain management and medication timing
Supporting incision care awareness
Creating a comfortable recovery space (pillows, hydration, essentials within reach)
Infant Care Support
Bringing baby to parent for feeds to reduce unnecessary strain
Assisting with positioning for feeding that protects the incision
Diapering, burping, and soothing baby between feeds
Supporting skin-to-skin in comfortable, safe ways post-surgery
Feeding Support
Helping find comfortable breastfeeding positions (side-lying, football hold, laid-back)
Offering guidance with latch and feeding rhythms
Supporting bottle feeding if needed (paced feeding, partner involvement)
Ensuring parent is nourished and hydrated during feeds
Emotional Support
Holding space to process the birth experience (especially if unexpected)
Normalizing feelings around cesarean birth and recovery
Offering reassurance, encouragement, and non-judgmental support
Watching for signs of postpartum mood challenges and gently guiding toward support if needed
Household Support
Light household tasks (laundry, dishes, tidying common areas)
Meal prep or simple nourishing snacks
Keeping frequently used items accessible to minimize movement
Supporting organization of baby and recovery supplies
Partner & Family Support
Guiding partners in how to support recovery and baby care
Helping distribute responsibilities in a realistic, sustainable way
Creating space for partner rest and confidence-building
Education & Guidance
Teaching newborn care skills and building confidence

Doulas support cesareans by discussing your options early, helping you determine your birth preferences and providing in person support leading up to, during, and after your birth. Ultimately, the goal we have for all of our clients is to help them feel as prepared, respected, and nurtured as possible during all birth, including cesarean section!
